$299 / year per adult
$199 / year per child (up to 14 years old)

- 2 Hygiene Visits (cleaning & exams)
- Comprehensive exams at each visit
- All needed digital X-rays
- Fluoride treatment (if needed)
- Oral cancer screening
- 20% off all additional dental treatments (excludes orthodontic care)
SMILE ENVY PLATINUM PLUS
$399 / year (all ages)
INCLUDES ALL PLATINUM BENEFITS, PLUS:
- All benefits and discounts listed above
- Additional 10% off orthodontic treatment (braces) during the agreement year
